Patient appreciation day Bowl-a–thon registration form

Participation rules

1. Please be at the Bowling alley 30 minutes prior to the start time in order to be ready to start the game @ 11:00 (put the shoes on, be assigned to a table/lane, have your drink and snack and make the setups for the game).
2. No more than 2 family members can play. Spots are limited. We have the capacity for 150 people and we would like to allow as many families as we can to participate. Bring your whole family with you to encourage the players, have a drink and a snack, and socialize with other members of the community.
3. Everything is totally free (shoe rental, game time, drinks and snacks). You are our guests!
4. We will have time for approximately two games. Do not leave at the end of the two games. We will have prizes for bowlers, including a $250 grand prize.
5. We will have a raffle with approximately 50 door prizes (Mary Kay baskets, bottles of wine, therapeutic massages, gift cards, movie tickets, and much more). In order to enter, if you so choose, you have to buy tickets. One ticket is $1 and entry is limited @ 10 tickets per person. All the proceeds, including our 100% matching donation, will go to support burn care and burn recovery programs @ Firefighters Burn institute in Elk Grove.
